Gynecomastia Surgery Recovery: Week-by-Week Timeline and Grades
How chest reduction heals week by week, what Simon grades change about the operation, and when the gym, shirts and swelling are realistic.

If I can feel a firm disc under the nipple, liposuction alone will not flatten it. I would rather remove that gland through a small areolar incision than leave you with a puffy nipple after the fat is gone.
— Dr. Aamod Rao
Most men are back at a desk within a few days of gynecomastia surgery, in a compression vest, and are walking the same day. Upper-body training usually waits four to six weeks. The chest you see at two weeks is swollen. The chest that counts is the one at three months, when the skin has stuck down and the swelling has left. How much of that wait you have depends on whether we removed fat, gland, or skin as well.

Before any date on a calendar, the chest has to be diagnosed. A soft, spread-out fullness in a man who is above his ideal weight may be fat, and weight change comes first. A firm mound under the nipple, including in a lean man, is gland. Gland does not leave with diet. The difference is explained in gynecomastia or chest fat. Medicines, hormones and anabolic steroid use can drive gland growth, so we ask about them and, when the history suggests it, check before operating. Surgery on an untreated hormonal cause is how recurrence happens.
Grades, and why they change the operation
Surgeons often use the Simon grades to describe what the operation must include.
| Grade | What we see | What the operation usually involves |
|---|---|---|
| I | Small mound, no extra skin | Gland excision, often with liposuction |
| IIa | Moderate breast, no extra skin | Liposuction plus gland excision |
| IIb | Moderate breast with some loose skin | Liposuction, gland excision, and a judgement on whether skin will retract |
| III | Large breast with clear skin excess | The above, plus a skin-tightening pattern and a longer scar |
Grades I and IIa are the operations with a short scar at the lower edge of the areola, sometimes with liposuction entry points that heal as dots. Grade III is a different scar conversation, because skin that is truly extra will not shrink onto the chest. We say that in the consultation, with you standing and with the skin pinched.
The first week
You go home in a compression vest. It holds the skin to the chest wall so fluid does not collect in the space where fat and gland were removed. Wear it as directed, including at night. Most vests are on for about four to six weeks, off only to wash.
Pain is usually soreness and tightness, stronger when the gland was dissected under the nipple. A small drain is used only if the field needs it. Desk work is often possible in three to five days if you can keep the vest on under a shirt. Site work, riding a bike hard, or a gym session is not. Keep the areolar dressing dry until you are told you can shower.
A little bruising and a flat, numb nipple are expected. A rapidly enlarging, tense, painful side is a haematoma and needs a same-day review.
Weeks 2 to 4
Swelling makes the chest look fuller than the final result, and one side often swells more than the other. Firmness under the skin, almost like a ridge, is scar and swelling in the liposuction layer. It softens. Nipple sensation may be dull, sharp, or itchy as the nerve recovers. Both patterns occur.
You can walk as much as you like. You should not bench press, do pull-ups, or carry heavy bags on the operated side. The vest is still doing the work of a dressing even when the incision looks closed.
Weeks 4 to 6, and the third month
Light cardio that does not bounce the chest may be allowed earlier. Loaded chest and shoulder work is commonly cleared between four and six weeks, and we would rather delay it than have a seroma or a stretched scar. By three months the contour is readable in a fitted T-shirt. Small irregularities can still soften after that. A remaining puff directly under the nipple at three months is different from swelling: that is a reason to be examined, because a rim of gland can have been left, or a scar ball can be sitting in the same place.
The operation itself is described in gynecomastia surgery in Mumbai. Puffiness that is mostly the nipple, in a lean chest, is covered in puffy nipples in men.
Frequently asked questions
A proper vest is visible if the shirt is very thin. Most men use a slightly heavier shirt or a layer for the first fortnight and then forget it is on. Taking it off early to hide it is how fluid collects.
Walking immediately. Lower body and gentle cardio when you are comfortable, often in the second or third week. Chest, shoulders and heavy lifting after the pocket has stuck down, commonly four to six weeks, and only after you are seen.
If the gland was removed and the cause has settled, recurrence is uncommon. It is more likely if a hormonal driver or steroid use continues, or if substantial weight gain replaces fat on the chest. We would rather find that driver before surgery.
Only when the skin is truly extra, which is Simon IIb or III, often after a large weight loss. Mild looseness in a younger man with elastic skin often retracts once the volume is gone. We will tell you which group you are in while you are standing in front of the mirror, not from a message.
Have the chest examined before you book a theatre date
A photograph is a start. The exam that matters is whether there is a disc of gland, whether the skin will shrink, and whether a blood test belongs in the plan. Male chest reduction at The Sculptique Aesthetics is planned around those three findings.
