Breast Implants Over or Under the Muscle? How We Choose in Mumbai

What subglandular, submuscular and dual-plane placement change about look, rippling, movement and recovery, and how tissue cover decides the plane.

I do not start with a favourite plane. I start with how much of your own tissue will cover the implant. That measurement decides more about a natural edge than the volume you have in mind.

— Dr. Aamod Rao

The implant can sit above the chest muscle, under the gland only, or partly behind the muscle in a dual-plane pocket. Above the muscle is a shorter early recovery and a shape that moves less when you use your arms. Behind the muscle hides the implant edge when your own tissue is thin, at the cost of more tightness in the first weeks and some movement when the pectoral muscle contracts. Neither plane is the default. The right one is the one your tissue can cover.

Volume and profile are a separate decision. A 350 cc implant in the wrong plane will show its edge even if the number sounded right on a chart. Sizing is covered in the breast implant size guide. This article is about where that implant sits.

The three placements

Subglandular means the implant is behind the breast gland and in front of the pectoralis muscle. The muscle is not lifted. Recovery of arm movement is quicker. The implant follows the gland when you lie down, which some patients prefer. If the pinch of tissue at the top of the breast is thin, the upper edge and any fold in the implant can be visible. That is rippling, and it is more common in this plane. We discuss it in breast implant rippling.

Submuscular means most of the implant is behind the pectoral muscle. The muscle adds a layer over the upper half, which softens the edge on a slim chest. The lower part of the implant still needs gland or a planned release to sit in a natural fold. Early tightness and a longer wait before chest exercise are the trade.

Dual plane is the placement we use most often when a patient wants coverage at the top and a natural lower slope. The upper implant is behind muscle. The lower implant is against the gland, so the breast can sit in the fold rather than looking stuck high on the chest. There are degrees of dual plane, depending on how much the muscle is released. That is an intraoperative decision based on your fold and your skin, explained before surgery in plain language: how much muscle cover, and why.

Animation means the implant shifts when you tense the chest, for example in a press-up or a namaste held hard. It is more noticeable with a full submuscular pocket. It is a reason we talk about your training, not a reason to avoid muscle cover if your tissue is thin.

Who tends to suit which plane

A patient with a thicker pinch of breast tissue, who wants a shorter early recovery and trains her chest hard, may be well served in front of the muscle, provided we have checked that the edge will not show.

A patient with a slim upper chest, a wide gap between the breasts, or tissue that has thinned after pregnancy or weight loss, usually needs muscle cover at the top. Putting a high-profile implant above the muscle in that situation is how rippling and a visible rim start.

A patient who also has a descent of the nipple often needs a lift as well as an implant. The plane does not lift a nipple. That choice is set out in augmentation, lift, or both.

Recovery differences that matter at home

Over the muscle, arm use comes back faster, and many people describe tightness as breast tightness rather than chest-wall tightness. Under the muscle or dual plane, reaching up and sleeping on your side is uncomfortable for longer, and we delay chest and pushing exercises until the pocket has sealed, commonly around six weeks. The week-by-week pattern for augmentation is in breast augmentation recovery.

The plane can be changed later, during an implant revision, if thinning, rippling or a capsule makes the original pocket the wrong one. It is better chosen correctly the first time.

Frequently asked questions

Only when the problem is thin cover. On a breast that already has enough tissue, muscle cover can make the upper pole look tighter than you wanted, and it can move with exercise. Natural is a match between tissue and plane.

They change the first six weeks, not your long-term training. Once the pocket has healed, most patients return to chest work. A small number notice animation and prefer to keep heavy pec work lighter. We talk about that before you choose the plane.

The incision and how the gland is handled matter more than whether the implant is in front of or behind the muscle. Breastfeeding after augmentation is discussed in can I breastfeed after breast augmentation.

Yes. Revision surgery can move an implant from in front of the muscle to a dual-plane pocket, or the other way, when the original plane is causing a visible edge, animation you dislike, or a capsule. It is a second operation, with its own recovery.

Choose the plane at consultation, not from a photograph

Bring a photograph of the upper-chest look you want, and be ready for a pinch-thickness check. We will tell you which plane your tissue supports, and we will say so if the look in the photograph needs more cover than your chest can give without muscle. Breast augmentation at The Sculptique Aesthetics starts with that measurement.
