Rhinoplasty Swelling Timeline: Month by Month Until the Nose Settles

When the cast comes off, when bruising fades, and why the nasal tip keeps changing for a year, especially on thicker Indian skin.

The nose you see when the cast comes off is not the nose you will have at a year. I show patients that sequence before surgery, because the tip in particular stays swollen long after the rest of the face looks normal.

— Dr. Aamod Rao

A cast usually comes off around day seven. Bruising around the eyes is mostly gone by two to three weeks. The bridge looks closer to its shape by six to twelve weeks. The tip is the slow part: on many Indian noses, with thicker skin, the tip is still refining at twelve months, and sometimes at eighteen. That timeline is why a rhinoplasty result cannot be judged from a photograph taken in the first month.

Swelling is not a complication by itself. It is the way the skin and the lining respond to bone and cartilage work. What changes the schedule is skin thickness, whether the operation was open or closed, whether it was a first operation or a revision, and whether the septum was straightened at the same time.

Day 1 to day 7: the cast

You go home with a splint on the bridge and, often, soft packing or internal splints if the septum was treated. Breathing through the nose is limited. Sleep with your head raised. Do not wear glasses on the bridge. Bruising under the eyes can look dramatic even when the operation was modest, because the eyelid skin is thin and shows blood that has tracked down from the nose.

Pain is usually moderate and settles with the prescribed tablets. A fever, a sudden increase in bleeding, or a blow to the nose needs a call to the clinic, not a wait until the cast visit.

Week 2 to week 4: socially presentable, still swollen

The cast comes off at the first review. The nose looks swollen, and the tip looks rounder and higher than it will later. This is the week patients panic, and it is the week we ask them not to judge the result. Bruising can be covered. Swelling of the tip cannot.

Most people feel comfortable being seen at work or at a family event somewhere in weeks two to three, with the understanding that close photographs will still show swelling. Blowing the nose, heavy spectacles, and sleeping face-down are still restricted. Saline rinses, if you have been shown them, keep the lining comfortable. They do not shrink the tip.

Month 2 to month 3: the bridge declares itself

By two to three months the upper half of the nose, the bridge and the bones, has usually lost most of its swelling. Profiles start to look like the plan. The tip and the supratip, the area just above the tip, stay fuller. On a thin-skinned nose this phase is shorter. On a thick-skinned nose, which we see often in ethnic rhinoplasty for the Indian nose, the tip holds fluid longer and the definition arrives later. Steroid injections are sometimes used for a stubborn supratip swelling. They are a clinic decision, not a home treatment.

Month 6 to month 12: the tip

At six months a first rhinoplasty on thinner skin often looks close to final. At twelve months we are looking at scar inside the tip settling, and at skin redraping over the new cartilage framework. Revision surgery, if it were ever needed, is not planned on a three-month photograph. Operating again before the skin has settled makes the next operation harder.

A practical way to watch this without obsessing is a set of photographs in the same light: front, both sides, and the base of the nose, at one month, three months, six months and one year.

What makes swelling last longer

FactorWhat it does to the timeline
Thick tip skinTip definition arrives later, often past six months
Open rhinoplastyA little more early tip swelling than a closed approach, same end point
SeptorhinoplastyMore internal swelling and congestion in the first weeks
Revision rhinoplastyMore swelling and a longer wait before judging the tip
Salt, heat, exercise, alcohol in month oneSwelling fluctuates day to day
A knock or glasses on the bridgeCan restart swelling over the bones

Open and closed approaches are compared in open versus closed rhinoplasty. Breathing and shape done together are covered in septorhinoplasty in Mumbai. A wider recovery overview is in rhinoplasty recovery time.

Frequently asked questions

Most patients are happy to be seen once the cast is off and the worst eyelid bruising has faded, often in the second or third week. The tip will still look broader than the final result.

The tip skin is the thickest skin on the nose and it swells the most. Roundness at day seven is expected. Definition comes as that fluid leaves, which is measured in months, especially if your skin is thick.

Not resting on the bridge while the bones are healing, commonly for about six weeks. If you must wear glasses, we show you how to tape them off the bridge. Contact lenses are usually fine once any eye swelling allows them.

Not in the first months, unless there is a clear early problem such as a shift after an injury or a breathing issue that is not settling. Shape revisions are a conversation after the nose has matured, usually around a year.

Plan the nose and the wait

If you are comparing clinics on price alone, also compare who will see you at one week, one month and one year. The operation is a day. The swelling timeline is the part that decides whether you are happy with it. We plan rhinoplasty around your skin thickness, not around a cast-off photograph.
